Residential/Day Case Management

The Moore Center provides Residential/Day Case Management to more than 200 individuals, with an intellectual and or developmental disability or an Acquired Brain Disorder, who are 21 years or older.  Individuals are served outside of their family homes with paid providers on a 24/7 basis.

Residential/Day Case Managers evaluate the need for and coordinate services, prioritize the level of service coordination needed and document outcomes.  Case Managers negotiate services for the client that would be the most appropriate and cost effective while continually evaluating the quality of services delivered for creating opportunities for a good and meaningful life.
