Moore Center Services offers a range of services, each tuned toward creating the best possible scenario for the respective clients. Typically, potential clients are referred to our agency through doctors or schools. Family Support

The Family Support department is a major component of Moore Center Services. Serving over 900 families in the Greater Manchester Area, Family Support far and away hits the broadest spectrum of clients and needs in the agency.

Fully staffed with case managers, a behaviorist, an early intervention coordinator, and two transition coordinators (who aid with a client moving away from school services), the department seeks to offer its families as much support as possible.  This support can come in a variety of ways: financial, through the respite program; social, through Fun n' Friends and assorted recreation events; and informational, with case managers aiding family members in negotiating the challenges of Medicaid and procurement of services.

Headed by Ilene Wheeler and managed by Dawna Garofalo, Family Support is committed to supporting the needs of our clients.    The Family Support Advisory Council is comprised of parents and guardians and meet the second Tuesday of every month.  For general information about Family SUpport or to learn more about the council, please call 603-206-2701.

Moore Options for Seniors

The largest and fastest growing demographic in New Hampshire is people over the age of 65. Studies show that when given a choice seniors want to remain in their homes and communities for as long as possible, but many of them may need help with simple household chores, while others need a more complex continuum of care.   Moore Center Services offers initiatives that enable seniors to remain independent.

Most seniors want to live independently and remain in their communities as long as possible. As individuals age, however, the ability to live on their own is compromised by changes in their physical and mental health and a lack of resources and companionship.

Now available from Moore Center Services, one of New Hampshire's largest most innovative non-profits, is a program that helps create a safe and secure environment for seniors and enables them to continue functioning as productive members of their communities.

Children's Services

Moore Center Services coordinates and oversees the provision of family-centered, home-based
early supports and services to eligible infants and  toddlers (birth to three years of age) in the eight communities served by our agency.  Eligibility generally is based upon an established condition, a developmental delay or a risk of a significant delay.   Transition assistance is provided to families whose children begin special education services in their schools at the age of three years.  Call for 603-206-2809 for more information on ESS.

Parent-to-Parent (PTP) matches parents with other parents who share similar experiences and feelings about raising children with disabilities.  PTP also offers parents support meetings,  workshops on disability issues and trainings for parents who want to become a supporting parent to others. In-Home Supports (IHS) is a Medicaid-funded program for eligible children 0-21 that brings supports and services into a family’s home.  For information and eligibility criteria call 603-206-2825.

Individual Development Services

IDS provides day services and employment services for people with developmental disabilities and brain injuries from young adults to seniors.  IDS offers individualized services designed to support people to be contributing independent members of their community, to determine safe and healthy life choices and to establish meaningful relationships with peers, family and friends.

IDS strives to create a meaningful day  through a curriculum-based skill building program and career development program enhanced by community activities to expand on the skills learned in the classroom.   In addition, IDS strongly promotes employment for anyone seeking paid employment.  The program has a record of successfully generating good job matches, and for supporting people once they are employed.

Family Directed Services

This program is designed around the adult (21 and over) client living at home. The family
receives a stipend to assist their loved one with daily living, Personal Care Services, behavioral, safety and/or health related skills.  Moore Center Services, Area Agency VII, working together with our clients, their guardians and families, will assure that the agency’s Family  Directed programs comply with all relevant, applicable state and federal laws and regulations.

Family members currently have the option of  providing personal care services directly or hiring an
individual/agency (called subcontractor) or requesting that the area agency hire staff to come into the individual’s home and provide services. Reimbursement is made on a monthly basis to either the family member directly or to the subcontractor or both depending upon the needs of the consumer and the funds available.

Consolidated Services are coordinated for the benefit of the client; a Family Manager is delegated by the family/guardian. The Family Manager works within the client’s budget to engage the best possible services and to ensure that goals are being addressed, monitored and documented.  These services can include Service Coordination, Residential and Day Services and Respite. Consolidated programs are all individual focused and all certified individually.
