Kinship Care
The Moore Center’s Kinship Care program enables families to be compensated for caring for a loved one at home, in lieu of having them placed in a nursing home.
Kinship Care Program Requirements
To participate in Kinship Care an individual must be qualified for Medicaid and the Choices for Independence program through ServiceLink. You can visit the ServiceLink website or call them at 866-634-9412. You can also contact your local Service office for more information.
Here are the steps to participate in our Kinship Care program:
- Get qualified for Medicaid and the Choices for Independence program
- Once you’re qualified, they’ll assign you to a case manager
- Have your case manager contact The Moore Center to enroll in the Kinship Care program

How do I Qualify?
To start the process and determine if you are eligible for Kinship Care, the first step is to contact a ServiceLink office near you. They will determine if you are eligible for Medicaid and the Choices for Independence (CFI) program. They determine this based on care needs and financial assets of the person in need of care.
Once eligibility is determined, you will be assigned to a case manager and we will get notification and authorization directly from them. Once we have that in hand, our Program Coordinator will give you a call to set up your intake meeting to get you started.
In general, candidates who qualify for funding for Kinship Care:

Meet both the financial and clinical eligibility requirements for the New Hampshire Bureau of Elderly and Adult Services (BEAS’) Choices for Independence (CFI) Program

Are declining and may be losing the ability to live alone

May already be living with a family caregiver and are eligible for the CFI program due to increasing care needs

May be planning to leave a nursing facility but do not want to, or are unable to, live alone.

Are able to use the telephone effectively during an emergency

Are not in need of continuous skilled nursing care, except during periods of acute illness

Can be left alone and unsupervised for a portion of the day

Are not a danger to themselves or others

Have medical diagnoses and conditions that can be safely managed in a private home setting
