We’re Hiring Case Managers!

These rewarding positions don’t come around often. If you’re a human services professional who’s tired of a routine, this is the position for you. It’s a challenging, yet very rewarding opportunity to help families and their loved ones who are affected by a variety of intellectual disabilities get the services they need. This is a dynamic position that includes meeting with families to evaluate needs, and working with a variety of providers to ensure those needs are met. 

You’ll be the advocate for your clients with all of their providers, striving to get them the best services available. You’ll also work with and learn from clinical experts in a person-centered environment, acting as your client’s champion. You’ll witness their day-to-day successes, as well as the larger transformations that happen over time as they benefit from the services you incorporate into their plan. 

The ideal candidate will be someone who is compassionate, well-organized, efficient and creative in evaluating each client’s need for services. In addition to providing service coordination, you’ll also be responsible for documenting progress and/or continued need. You’ll be working in an environment where you’re empowered to make decisions and take action. 

As a Case Manager you’ll typically have responsibility for 25 to 40 clients, depending on their needs, and here are some of the key position responsibilities:

  • Work with various clinical professionals to develop assessments and evaluations
  • Create service plans and budgets to support your clients’ needs
  • Negotiate needs for client-specific services in a cost effective way and evaluate the quality of services delivered
  • Work across a variety of providers to meet your client’s needs
  • Develop clinical skills and knowledge
  • Find creative solutions to challenging situation

Ideal candidates would typically have experience in one or all of the following areas:

  • Case Management
  • Mediation/Negotiation
  • Social Services
