Essential Responsibilities: System Navigation • Identify the needs and support required for individuals by accessing appropriate assessment tools as needed. • Provide information and assistance to individuals and their support teams on options to meet their needs. • Work collaboratively with community service partners to facilitate connections on behalf of the individual • Facilitate transition planning as required between waivers or services. • Inform individuals of the appeals process Person-Centered Planning: • Identify individual needs/goals and preferences and champion those at all intersections of the process • Coordinate the service planning process as outlined in He-M 503. • Ensure all documentation is maintained, on time, and compliant with regulations outlined in He-M 503. • Support the individual to facilitate the choice and selection of service providers to deliver services, conduct outreach on behalf of the individual • Ensure adequate financial resources are secured for all requested services through budget development, accurate and timely authorization submission and room and board requests etc. • In advance of the annual service planning meeting, consult with the individual and team members to understand the individual’s satisfaction with services, progress on their goals, and to identify any risk factors and plans to minimize them, as outlined in He-M 503, when applicable consult with the Human Right’s Committee through the formal process as outlined in He-M 504. This role also oversees the tracking of departmental revenue, ensures the accuracy of authorization and billing information, and serves as the primary liaison between the departments and the Business Office to support efficient financial and operational processes. Client Description: Becca is a passionate, playful, and genuine 31- year-old woman. She loves her animals, and they sometimes help regulate her anxiety at home and in the community. When at a behavioral baseline, Becca enjoys social interactions with others, especially those close to her, and with whom she has formed relationships within her social circles. She does exhibit intense behavioral outbursts. Becca has a close relationship with her family and likes to socialize with friends whom she has maintained relationships with from Special Olympics'. Becca also attends equine therapy where she helps care for the horses and enjoys going out to eat and shopping. She is mobile but requires a wheelchair for long distances as she tires easily and is at high risk for drop seizures. She also utilizes a gait belt/harness for walking and transfers, as she has an extensive seizure history. Behavioral challenges along with ongoing seizure activity continue to be an issue for Becca's day-to-day living and functioning. She requires personal care support from her home care provider and staff but has been working on her independence with her continence and hygiene. He enjoys cooking and shopping for his lunches, He enjoys berry, apple, and peach picking, and then creating muffins and desserts for his family. Micah’s regulation benefits from 2 hours of physical movement daily, without it he easily becomes dysregulated. Schedule- Saturdays 9am-5pm (Flexible) Pay: $21/hr. plus mileage reimbursement Client is looking for someone who is fun, with a joyful sense of humor, someone who will drive and attend activities in the community. Client enjoys shopping, visiting friends, and volunteering at a pet center. She seeks a trusting relationship to assist her in getting around when using crutches or wheelchair gaining access to public places. She does need a high level of supervision. (CW) Schedule: 10am-3pm, 15 hours a week. Pay: $19/hr. with mileage reimbursement A 25-year-old developmentally disabled young man needs direct support staff to provide community-based services and support. One or more shifts are available for full or half days on a part-time basis. Duties include Job coaching at various workplaces; taking client to and from therapy appointments and community-based activities; and working with the client on functional skills and community activities such as assistance with speech practice, learning simple job skills, exercise, grocery shopping, eating out, kitchen skills, etc. Client lives in Bedford, NH, and his home is a non-smoking home. He is autistic with intellectual, speech, and language impairments, but he is very social, independently mobile, and demonstrates compliant behavior. Due to intellectual disability, he does require regular monitoring and guidance. The work is rewarding and in pleasant home and work environments. Candidate must have reliable vehicle and auto insurance and pass a complete background check. References are required. Part Time - 5-10 hours weekly Pay: $20.33/hr. Sam is a 29-year-old gentleman who is diagnosed with autism and lives with his father. Sam is nonverbal, however, communicates by using hand gestures, minimal ASL signs, and head nodding. He may gently guide staff by the arm and point to express his wants and needs. Sam and his father are seeking a DSP to provide 10 hours of weekly support within his residence. Sam's father is flexible with how the number of hours is divided throughout the week. Sam enjoys music from the 1970's and 1980's. The Rolling Stones are a favorite household band. Sam will rock and dance to music. Sam has a laptop and has been able to navigate the Internet to watch his favorite videos or listen to music. He navigates YouTube and watches videos by himself. Sam likes the Wiggles, Thomas the Tank Engine, or Richard Scarry videos. Sam carries a "jump rope" or shoestring with him at all times. He uses this as a sensory activity. Sam also carries a picture book with him everywhere he goes. Sam does best with a structured routine. Sam follows a specific sequence of getting ready each day; however, Sam often gets stuck between steps and has difficulty with transition. It is important for staff to be patient and gently encourage Sam to move on to the next step in his routine. This ensures Sam's success. Sam is independent with some activities of daily living (ADLs); however, he may need consistent verbal cues to stay on track. Sam may require minimal hands-on support with personal care and meal preparation. PAY: $20/hr.
