The Family Support Council’s Financial Assistance Program

Did you know there is financial assistance available through The Moore Center Family Support Council for services and items that will help improve the quality of life for individuals with developmental disabilities or Acquired Brain Disorder? The Family Support Council works closely with The Moore Center’s Family Support Services staff to help assess the needs and determine how best to support families. A Moore Center client family is eligible to request assistance if a person with a disability lives at home with a family member, or the individual resides independently and receives support from the family.

What’s Covered?

Below is a list of services and items that MAY be covered by the Moore Center Family Support Council’s financial assistance program. Only items not covered by Medicaid will be considered. Reimbursement is capped at $500 for many items and services. Financial assistance is subject to budget availability and approval by Family Support Staff. 

  • Therapist or physician recommended items:
    • Adaptive Equipment and Repairs, including Clothing and Footwear
    • Safety Equipment
    • Assistive Technology/Communication Devices
    • Home/Environment and Vehicle Modifications
  • Memberships to Therapy Classes and Gyms (therapeutic services)
  • Educational Materials
  • Supportive Services Evaluations

In addition, The Moore Center Family Support Council has funding available to assist with:

  • Crisis Intervention (food vouchers, heating fuel, utilities, etc.)
  • Parent/guardian Respite Services
  • NH Leadership Program – Institute on Disability
  • Family Support Conference Scholarships • Camperships (up to $600/year)
  • Legal & Educational Advocacy Support (school-related disputes, guardianship, power of attorney, special needs trust, etc.)

We Also Have Food Vouchers Available for Moore Center Families

The Moore Center’s Family Support Council has purchased food vouchers to assist families with food purchases. If you would like to receive a food voucher for your family, please contact your case manager as soon as possible. Vouchers are only available until the end of June 2024. 

To apply, families must provide the following information to their case manager by email, phone, or mail:

  • Your name
  • Moore Center client name
  • Requested items or services
  • Amount of money being requested
  • Documentation
  • List of other financial resources pursued
